Race to Own Clinical Copilot
OpenEvidence vs. ChatGPT for Clinicians
This is no longer a niche reference tool battle, it is a land grab for the doctor’s main working screen. OpenEvidence started with search, but once it reached roughly $300M annualized revenue by July 2026 on an ads model with about 90% gross margin, it became attractive to three different kinds of competitors, UpToDate defending paid clinical reference, Doximity and Abridge expanding from adjacent doctor workflows, and OpenAI using general purpose distribution plus healthcare specific products to move into clinician use cases.1 2 3 4 5 6
- UpToDate’s advantage is trust and hospital contracts, not speed of bottom up adoption. It had about $690M of 2025 revenue, more than 3M clinicians globally, and roughly 80% of revenue from institutions, then launched UpToDate Expert AI in October 2025 to keep that enterprise base from defecting to a free search first product.1 6 7
- Doximity and Abridge are coming from the opposite direction. Doximity bundled Ask, Scribe, and Dialer into a clinical AI suite for doctors already using its network and communications tools. Abridge added NEJM and JAMA into its note taking workflow, so a doctor can capture the visit and see supporting evidence without opening a separate research app.1 3 8 9
- OpenAI is the broadest threat because it can copy the free adoption motion at massive scale. By April 2026 it had both ChatGPT for Healthcare for hospital providers and ChatGPT for Clinicians, which means OpenEvidence is no longer just competing with medical publishers and doctor software vendors, but with the default AI product many clinicians already use off label.1 5 10 11
The market is converging toward an all in one clinical copilot that searches literature, writes notes, suggests codes, places calls, and plugs into the EHR. The winners are likely to be the companies that own daily clinician workflow, not the ones with the best standalone answer engine, which pushes OpenEvidence to keep expanding beyond search while defending its audience and content moat.1 2 3 4 8 9
