The key strategic issue is whether the first human setback lands on reprogramming as a whole, or mainly on one payload design. Life Biosciences is taking the most exposed route, using AAV to deliver OCT4, SOX2, and KLF4 into the eye, so any safety problem around uncontrolled cell state changes, immune reaction, or dose control would hit OSK programs first. Altos has publicly framed its work more broadly around rejuvenation and computation driven intervention design, which leaves room for non Yamanaka approaches that could look safer or easier to tune.

  • Life Bio is the category’s first real readout in humans. ER-100 received FDA IND clearance on January 28, 2026, and the company announced first patient dosing in June 2026. That means the earliest clinical signal will mostly test this exact stack, AAV delivery, eye tissue targeting, and OSK payload behavior, not every reprogramming method.
  • Altos has not disclosed a named clinical candidate, but it has publicly emphasized a wider search process. Its science materials point to partial reprogramming roots, while its Institute of Computation is built to discover and design rejuvenation interventions, suggesting a larger design space than simply carrying OSK into the clinic unchanged.
  • A useful comparison is NewLimit, which explicitly says the hard part is finding new reprogramming payloads and plans to enter its first human trial in 2027 with transient mRNA and non viral delivery work. That shows the field is already splitting between classic factor sets and newer payload engineering approaches.

The next phase of the market will sort companies by payload and delivery architecture, not by broad anti aging ambition. If OSK in AAV shows limits, programs built around alternative factor combinations, transient dosing, and heavier computational screening could gain credibility fast, and that is the lane where Altos appears best positioned to benefit.
