The real moat has shifted from writing the note to owning the next click after the note. Once speech to text and note drafting got good enough, the scribe itself stopped being scarce, so value moved to deeper EHR integration, coding, pre charting, billing support, and distribution inside the clinician workflow. That is why the leading vendors are all stretching from ambient capture into broader clinical and revenue cycle jobs.

Freed and Heidi first won with simple, self serve scribing for small practices, where a doctor could buy a $99 per month tool without a long IT process. But that motion caps out because small clinics have few seats, thin budgets, and fragmented EHRs that make deeper automation harder to scale.

Abridge and Ambience pushed upmarket by embedding into hospital workflows. Abridge reached an estimated $100M ARR by May 2025 through Epic and Athenahealth distribution, while Ambience reached an estimated $30M ARR by May 2025. Their pitch is not just note creation, but producing billing ready documentation and helping health systems capture more revenue.

The newer battleground is bundle breadth. Freed is adding search, coding, and receptionist style workflows around its clinic base, while Heidi layers enterprise sales on top of PLG and expanded to larger systems and markets outside the U.S. Estimated scale now reflects that shift, with Freed at $33M ARR in August 2026 and Heidi at $50M ARR by March 2026.
From here, the winners are likely to look less like standalone scribes and more like workflow suites for specific care settings. In small practices that means an all in one front office and documentation stack. In hospitals it means deeper EHR embedded copilots that turn conversation into notes, codes, orders, and cleaner claims.
